Health Care Coverage for Members With Florida Medicaid

Community Care Plan (CCP) has a contract with the Florida Agency for Health Care Administration to provide health care services to people with Medicaid in Broward County. This is called the Statewide Medicaid Managed Care (SMMC) Program.

Community Care Plan members enrolled in our Medicaid plan can contact our team for help with many services.

If you are a Community Care Plan Medicaid member, please call us at 1-866-899-4828 for help with:

  • Finding a provider
  • Choosing or changing your Primary Care Provider (PCP)
  • Finding out if a service is covered
  • Getting referrals
  • Replacing a lost ID card
  • Planning care for the birth of a new baby
  • Explaining plan changes or benefit options
  • Answering general plan questions

Contacting Member Services

Our team can assist you by phone at 1-866-899-4828, TTY: 1-855-655-5303, Monday through Friday, 8 a.m.-7 p.m.

When you call, please remember to have your identification card (ID card) with you so we can help you more easily. (If your ID card is lost or stolen, your Member Services Representative will help you replace it.)

Using MyChart, Your Member Portal

As a Community Care Plan member, you have access to MyChart, our member portal. This is a secure self-service online portal where members can get personalized information about benefits and services. You can also communicate with our Member Services team.

After you sign up, you can sign into the online portal with your confidential username and password. Once you sign into the member portal, you can:

  • Order an ID card
  • Change your PCP
  • Check the status of medical claims
  • Find an in-network doctor
  • Message your doctor
  • Check your test results
  • Ask for prescription refills
  • Make or change appointments

Managing Care With a Helping Hand

If you have a medical condition or illness that requires extra support and coordination, we may assign a case manager to work with you. Your case manager will help you get the services you need.

The case manager will work with your other providers to manage your health care. If you have a problem with your care or something in your life changes, let your case manager know and he or she will help you decide if your services need to change to better support you.

Community Care Plan (CCP) offers health care with pregnancy coverage for Medicaid members in Broward County. Our Birth, Baby, and Beyond program helps women get the prenatal care and support that is so important for a healthy pregnancy.

Birth, Baby, and Beyond Program?

Our program helps pregnant Community Care Plan members with everything from finding a doctor to birth planning to baby supplies and more. We will connect you with local experts in Broward County for prenatal and postpartum care.

Our Birth, Baby, and Beyond program includes more than just health care coverage. We help mothers with many services before, during, and after pregnancy.

If you are a Community Care Plan Medicaid member, please call us at 1-866-899-4828 for help with:

  • Finding a doctor for you and your baby
  • Making prenatal care and postpartum appointments
  • Getting educational information about caring for your baby and safety
  • Transportation to medical appointments
  • Getting baby supplies, food, prenatal classes, housing, and breastfeeding help
  • Making delivery plans

Our team of trained obstetrics (OB) nurses will be your guides to every important health step for you and your baby. This includes checking for any health risks to give you and your baby the right level of care.

Not Sure If You Are a Member?

If you are approved for Medicaid and not sure if you are a Community Care Plan member, you can speak with a choice counselor to find out which care plan you are enrolled in. If you have not picked a health plan for your pregnancy, you may have already been placed into a health plan.

How to Choose Your Pregnancy Care Plan

Once you are approved for Medicaid, you will be able to choose the right health plan for you and your baby.

The easiest way to do this is by calling the Choice Counseling Helpline toll-free at 1-877-711-3662.
A counselor will help you choose your care plan or switch to a plan provider.

 The Agency for Health Care Administration collects information from all plans on different performance measures about the quality of care provided by the plans.
