Community Care Plan (CCP) offers health care with pregnancy coverage for Medicaid members in Broward County. Our Birth, Baby, and Beyond program helps women get the prenatal care and support that is so important for a healthy pregnancy.  

If you are pregnant, this is a special time in your life. But we also know how hard it can be to keep track of all your prenatal care needs. That’s why we have a full team of experts ready to help our members every step of the way.

What is included in the Birth, Baby and Beyond program?

Our program helps pregnant CCP members with everything from finding a doctor to birth planning to baby supplies and more. We will connect you with local experts in Broward County for prenatal and postpartum care

Our Birth, Baby, and Beyond program includes more than just health care coverage. We help mothers with many services before, during, and after pregnancy.

If you are a CCP Medicaid member, call us at 1-866-899-4828 for help with:

  • Finding a doctor for you and your baby
  • Making early prenatal and postpartum care appointments
  • Educational facts and information about your baby's care, safety and breastfeeding
  • Getting transportation
  • Baby supplies, food, prenatal classes, housing, and breastfeeding assistance
  • Making delivery plans

Our team of trained Obstetrics (OB) nurses will be your guides to every important health step for you and your baby. This includes checking for any health risks to give and your baby the right level of care. You can also trust our team for friendly appointment reminders, helpful tips, or even just a good listener when you need one.

Not sure if you are a CCP Medicaid Member?

If you are on Medicaid but have not picked a health plan for your pregnancy, you may have already been placed into a health plan.  To find out if you are already a CCP member or with another health plan, you can call 1-877-711-3662 to speak with a Medicaid Choice Counselor.

Getting Medicaid for Pregnancy Coverage

If you are approved for Medicaid in Florida, you can receive affordable and expert care for you and your baby. An important step in that process is choosing the health plan for your pregnancy care.

How to apply for Medicaid in Florida

If you are pregnant, live in the state of Florida, and do not have health insurance, you can still receive health coverage for your prenatal and postpartum care through Medicaid.

You can apply for Medicaid coverage for pregnancy online. Apply through ACCESS or apply through Healthcare.gov .

Once you are approved for Medicaid, you can view your health plan options, compare plans, and choose the plan that is right for you on the Florida Medicaid Managed Care website.

How to Become a New CCP Medicaid Member

Once you are approved for Medicaid, you will be able to choose the right health plan for you and your baby. The easiest way to do this is by calling 1-877-711-3662 and speaking with a Medicaid Choice Counselor. This is when you will be able to request to join Community Care Plan.

New members join the CCP family all the time for their pregnancy health coverage. We are proud to help them with the care that moms and kids deserve.

Below are the steps you can take to become a CCP Medicaid member:

  • Make sure that you have been approved for Medicaid for your pregnancy. You can apply or check your enrollment status online at  ACCESS or at Healthcare.gov
  • You may have already been placed with a managed care plan. Call 1-877-711-3662 to speak with a Medicaid Choice Counselor and find out if you do or do not already have a health plan.
  • Visit the FL Medicaid Managed Care website and enter your zip code. This will show you which managed care plans are available to you in Broward County.
  • To choose or to change your health plan or managed care plan, you have a few options:
    • By Phone: Call 1-877-711-3662 (Mon.-Thur. 8 a.m.-8 p.m. & Friday 8 a.m.-7 p.m.) and a Medicaid Choice Counselor can help you choose a plan that fits your needs, such as Community Care Plan.
    • Online: You can view or make changes to your health plan online. The first step is to Create your account if you don't have one.
