Community Care Plan’s Medicaid and Florida Healthy Kids’ members qualify to receive a reward for becoming fully vaccinated by receiving two doses of the Pfizer-BioNTech or Moderna or one dose Johnson & Johnson COVID-19 vaccines.

How do I get the reward for being fully vaccinated for COVID-19?

After receiving receive two doses of the Pfizer-BioNTech or Moderna or one dose of the Johnson & Johnson (Janssen) COVID-19 vaccines, members or parents/guardians should send an email to CCPreward@CCPcares.org with the following info:

Member’s first and last name
Member’s date of birth
Member or Parent/Guardian’s cell phone number

When will I receive the reward for being fully vaccinated for COVID-19?

Once we confirm that the member is fully vaccinated, a text message with the reward will be sent to the cell phone number provided. Rewards are usually sent within four weeks of CCP confirming that the member is fully vaccinated for COVID-19.
