How or where can I get my child’s Community Care Plan – Florida Healthy Kids ID card?

There are a few ways to get your child’s ID card:

  • View your child’s ID card in MyChart, our secure member portal, that is available online or by mobile app. If you want a physical card, you can also send us a message through MyChart.
  • Call our Welcome Team at 1-866-384-2926 Monday-Friday between 7:30 a.m. - 7:30 p.m ET and we will have one mailed to you.
  • Email us at ccp.customersvc@ccpcares.org and we will have one mailed to you.


How do I change my child’s Primary Care Provider (“PCP”)/doctor?

There are a few ways to change your child’s assigned PCP:

  • Send the change through MyChart, our secure member portal, that is available online or by mobile app.
  • Call our Welcome team at 1-866-384-2926. We can also help you find another PCP for your child.
  • Email us at ccp.customersvc@ccpcares.org.


I need to change my child’s name, address, and/or phone number. What do I need to do?

  • To change your child’s name or address, please call Florida KidCare Monday-Friday between 7:30 am and 7:30 pm ET at 1-888-540-KIDS (5437) (TTY: 1-800-955-8771) www.floridakidcare.org
  • To change your mobile phone number, email, or how we contact you, please call our Welcome Team at 1-866-384-2926 / TTY / TTD 1-855-655-5303.


Why do I need to answer the Health Risk Assessment or “HRA” for a child?

The HRA is a survey that helps us take better care of your child’s health. The HRA only takes a few minutes, and your answers are kept confidential.

  • You can find the HRA in the Welcome Kit that was mailed to your child. A postage paid envelope was also included, so it is free to mail the HRA back to us.
  • You can also find the HRA on our ‘Documents/Forms’ page.
  • If you would like to complete the HRA electronically, you can do so online or in the MyChart mobile application.


Is my child’s doctor or specialist in-network? How can I find out?

  • You can find our in-network doctors in our Provider Directory. You can also download or print the directory.
  • If you are unable to find the doctor you are looking for, please call our Welcome Team 1-866-384-2926 / TTY / TTD 1-855-655-5303 and they can help you find a doctor or provider. They can also update your assigned Primary Care Provider (“PCP”).


Where can I find what my child’s plan covers or what my child’s benefits are?

  • To learn about your child’s plan and benefits, please review the Member Handbook. You can view, download, or print the handbook.
  • If you have questions about your child’s benefits or would like a copy of the benefit handbook mailed to you, please call our Welcome Team at 1-866-384-2926 / TTY / TTD 1-855-655-5303.


Do you provide Dental benefits?

  • No, we do not provide dental benefits. Your dental plan is assigned to you by Florida KidCare. If you need to know what dental plan your child is assigned to, please contact our Member Services Team at 1-866-930-0944/ TTY/TDD 1-855-655-5303 or contact Florida KidCare directly.
