Not sure what the next step is? To help you get the most of your plan, here is a short list of important things to do.

  • Read the Welcome Letter and Privacy Notice provided in the Welcome Kit that you received in the mail.
  • Review the new ID card from the Welcome Kit.
  • Make sure that your name, date of birth, and Primary Care Provider (PCP) information is correct.

  • Put your ID card in a safe place.
  • Expect a call from our Member Welcome Team to get you learn about your benefits, help you change your PCP, and much more. If you have not received a call within two weeks of joining the CCP plan, please call us at 1-866-384-2926 / TTY / TTD 1-855-655-5303/li>
  • Fill out the HRA form and Medical release forms.
  • By paper with the HRA that was mailed to you in your welcome kit. When you are done, please return it with the free, postage-paid, self-addressed envelope that was also in the envelope.
  • Electronically through the MyChart member portal at any time.
  • Visiting our HRA webpage and submitting your information.
  • Call your PCP and if you have not had your annual check-up in the last year, schedule an appointment with your PCP.
