How to Choose Community Care Plan

By Phone:

Call the Choice Counseling Helpline toll-free at 1-877-711-3662 TDD 1-866-467-4970, Monday through Thursday, 8 a.m. to 8 p.m., and Friday 8 a.m.-7 p.m.

A counselor can help you choose Community Care Plan, change from a current plan to Community Care Plan, and answer questions.

Before you can enroll, you must be approved for Medicaid. Applying for Medicaid is easy.

Medicaid Eligibility:

Medicaid provides medical coverage to low-income individuals and families. Eligibility in Florida is determined either by the Department of Children and Families (DCF) or the Social Security Administration (for SSI recipients). DCF determines Medicaid eligibility for:

  • Parents and caretakers relatives of children
  • Children
  • Pregnant women
  • Former Foster Care Individuals
  • Non-citizens with medical emergencies
  • Aged or disabled individuals not currently receiving Supplemental Security Income (SSI)

How to Apply for Medicaid

You can apply for Medicaid coverage online.

Apply through Healthcare.gov

Once you are approved for Medicaid, you can view your health plan options and choose the right one for you at flmedicaidmanagedcare.com.

Florida Healthy Kids

Florida Healthy Kids is a comprehensive health plan that helps parents with children ages 5-18 afford high-quality medical and dental care for their kids. Parents do not need to be eligible for Medicaid to be approved by Florida Healthy Kids.

Here are the requirements to qualify for Florida Healthy Kids and instructions on how to apply.

Florida Healthy Kids Eligibility

To qualify for low-cost coverage, a child must:

  • Be between age 5 and 18
  • Be a U.S. citizen or qualified non citizen
  • Meet income eligibility requirements

How to Apply for Florida Healthy Kids

Applying for Florida Healthy Kids is easy and can be done in two ways.
