Community Care Plan (CCP) has a contract with the Florida Agency for Health Care Administration (AHCA) for their Statewide Medicaid Managed Care (SMMC) Program.

CCP provides health care services to people with Medicaid in Florida, including serious mental illness (SMI), in 19 counties:

If you have been approved for Medicaid and are a Community Care Plan member, this page has helpful information about the resources available to you.

Community Care Plan Offers More Than Doctor Visits

At Community Care Plan, we give you more than doctor visits, prescriptions, and hospital care.

We have expanded benefits to help keep you and your family healthy.

Members can earn rewards for the following:

  • Before Baby & Beyond: This program supports women during pregnancy, offering prenatal care and support for a healthy pregnancy.

Support for Chronic Conditions

Community Care Plan offers programs to help members with chronic conditions. Our Concierge Care Coordination (C3) programs, like case and disease management, are here to support you and your caregivers. Our goal is to help you follow your care plan, get healthier, and improve your life.

Need Help with your Medicaid plan? Our Member Services department can answer your questions.

Community Care Plan members who are enrolled in our Medicaid plan can contact our team for help with many services.

Please have your identification card (ID card) ready when you call. This will help us help you faster. (If your ID card is lost or stolen, our Member Services team will help you get a new one.)

Community Care Plan Cares About You

If you have any concerns with your children’s physical, occupational, or speech therapy services, please call Yvie Noel at 954-622-3272.

If you are a parent of a child who is getting Private Duty Nursing (PDN) services in your home, we encourage you to report all failures of the home health agency to provide your child’s authorized PDN services to:

  • Your Care Coordinator on their direct line, or  
  • After business hours (Monday to Friday 8:30 am to 5:00 pm), please call 1-866-899-4828 (press 1 for Medicaid and then 3), or
  • Our dedicated email address: PDNfailurenotification@ccpcares.org.

 

CCP Cares MyChart: Your Member Portal

As a Community Care Plan member, you have access to CCP Cares MyChart, our member portal.

This is a secure self-service online portal where members can get personalized information about benefits and services. You can also communicate with our Member Services team.

After you sign up, you can sign into the online portal with your confidential username and password. Once you sign into the member portal, you can view you or your child’s member ID card, change primary care providers, search for doctors, and more.

Not Sure If You Are a Member?

If you are approved for Medicaid and not sure if you are a Community Care Plan member, you can speak with a Choice Counselor to find out which care plan you are enrolled in. If you have not picked a health plan for your pregnancy, you may have already been placed into a health plan.

Additional Resources

The Agency for Health Care Administration collects information from all plans on different performance measures about the quality of care provided by the plans. The measures allow the public to understand how well plans meet the needs of their members. To see the plan report cards, please visit floridahealthfinder.gov/HealthPlans/search.aspx. You may choose to view the information by each plan or all plans at once. Members can also compare the costs of common health care services based on their plan.
