Community Care Plan Celebrate Milestone of 25 Years Serving the Community

South Florida-based Community Care Plan (CCP), the health plan with a heart, is pleased to announce that this year marks a significant milestone of 25 years serving the community. Over the last 25 years, CCP has served as a health plan for hundreds of thousands of Floridians in government-sponsored programs, including Medicaid, Children with Special Needs, and Florida Healthy Kids, and as an Administrative Services Organization (ASO) for uninsured programs and employer-sponsored health plans.

Founded as South Florida Community Care Network by safety-net hospital systems, the organization was approved as a Provider Service Network (PSN) and served as a fee-for-service health plan for these hospitals. Over the next decade, SFCCN served as a PSN for government-sponsored programs, such as Medicaid in Broward County and Children with Special Health Care Needs covered under the Children’s Medical Services division of Florida’s Department of Health in 16 other Florida counties.

In 2014, the plan infrastructure was established to serve as an ASO for safety-net hospital systems’ uninsured population and self-insured employer health plan as a not-for-profit LLC, and South Florida Community Care Network, LLC (SFCCN) was created. Two years later, Community Care Plan was created as the organization’s doing business as (DBA). In its second decade, CCP expanded by:

  • Obtaining an award for a five-year contract for eight counties for Florida Healthy Kids beginning January 2020
  • Obtaining an award for a six-year contract for 19 Florida counties (five regions) for Medicaid Managed Medical Assistance (MMA) Plus, including the serious mental illness population, beginning February 2025
  • Establishing a third-party administrative (TPA) division, SydCura, and securing contracts to serve as an ASO for the uninsured in Miami-Dade, Broward, and Palm Beach counties, as well as its first safety-net hospital client outside of Florida

 

While CCP was already active in the community, in 2021, CCP launched a virtual resource center, Heart, which stands for Health, Education, Access, Resources, and Tools. Three years later, with the help of our partners in the CCP led-Broward Maternal Health Collaborative, Broward Health, Urban League of Broward County, and Broward County Healthy Start Coalition, and with $850,000 in funding from the Health Foundation of South Florida, CCP opened the doors to our brick-and-mortar Heart community resource center in Lauderdale Lakes, Florida offering free events and resources, including:

  • Breastfeeding, childbirth, and doula classes
  • Support groups to connect with others
  • Job support for resumes and interviews
  • Computers with internet access
  • Food, exercise, and financial classes for all skill levels
  • Community Health Outreach Advocates to provide links to community resources
  • Childcare services while at the center
  • and so much more!

 

The center also includes a Broward Healthpoint Maternity Care Center that offers group prenatal care, family planning, routine prenatal care, prenatal care and ultrasound, and telemedicine.
