2025 marked a milestone year for Community Care Plan. We celebrated 25 years of service while advancing our mission to promote healthier communities and launched our health insurance marketplace division, 22 Health. Guided by our core principles and a commitment to high‑quality, accessible, and compassionate care, we continued to strengthen our impact across Florida. 2025 brought meaningful growth in our Medicaid, uninsured, and employer‑sponsored programs; improvements in quality outcomes; expanded community partnerships; and elevated service experiences for our members and providers.
