Community Care Plan Prepaid Visa®
Your Community Care Plan (CCP) Prepaid Visa® gives you an easy way to pay for some of our expanded benefits and get the rewards that you’ve earned. Funds are only deposited in your account and available to use when your request for an expanded benefit or reward is approved.
One Card for Each Address
- The CCP Prepaid Visa® card is automatically mailed to the member's address.
- One card is issued to the responsible party, adult, or eldest person at the member’s address. The cards are not issued to minors.
- Do not throw your card(s) out at the end of the plan year. The card will be active for the next year if you are still enrolled with Community Care Plan.
Your CCP Prepaid Visa® Card
- 1. Sign the Card
The CCP Prepaid Visa® is ACTIVE starting on your enrollment effective date. Sign the back of your card(s) as soon as you get them.
- 2. Earn Rewards
Some rewards, like the Health Snapshot, are automatically given to you once you qualify, and others you have to ask for. Learn more on our Rewards
You can swipe your card to pay for items anywhere Visa® is accepted.
- The card cannot be used at gas stations or for alcohol, tobacco, and/or lottery tickets.
- 3. Use Your Expanded Benefits
- You can use your card to pay for expanded benefits like:
- Fitness membership
- Housing assistance
- Medically related home care services/homemaker
- Swimming lessons
- Legal Guardianship (SMI members only)
- The card can ONLY be used for our expanded benefits and the businesses that provide them.

Expanded benefits marked with a * will be added to your CCP prepaid Visa when your request for this benefit is approved.
- 4. Check Account Balances
- Visit LH1ondemand.com to check your deposits and balances or call the phone number on the back of your card.
- You will only get emails letting you know if funds are added to your account if we have an email for you.
- Unused funds are removed from the account at the end of the year.
- 5. Over-the-counter (OTC) benefit
- The phone number on the back of the CCP prepaid Visa also lists OTC information.
- The OTC program is not part of the CCP prepaid Visa. Your OTC benefit is provided by Medline.
