    A managed care plan that provides services for the Managed Medical Assistance part (MMA) of the Statewide Medicaid Managed Care (SMMC) program.

  • In what areas does Community Care Plan serve Medicaid and Serious Mental Illness (SMI)?

    Community Care Plan serves Medicaid and Serious Mental illness (SMI) in Brevard, Broward, Charlotte, Collier, Desoto, Glades, Hendry, Indian River, Lee, Martin, Miami-Dade, Monroe, Okeechobee, Orange, Osceola, Palm Beach, Sarasota, Seminole, and St. Lucie counties.

  • What is a Primary Care Physician (PCP)?

    A primary care physician is a doctor who helps you with most of your health care needs.

  • Can I use Community Care Plan outside of the State of Florida?

    Yes, the necessary medical services that are not available in the State of Florida may have to be authorized. For additional information, you can call Monday through Friday from 8 a.m. - 7 p.m. at 1-866-899-4828 / TTY / TTD 711.

  • How do I change my doctor?

    Call us Monday to Friday, 8 a.m. – 7 p.m., at 1-866-899-4828 / TTY / TTD 711.

  • I need to see a specialist. How do I find one?

  • Can I go to a doctor not listed in the directory?

    Yes, only if it is medically necessary and has been authorized by Community Care Plan.

  • Who do I call with questions, concerns, or complaints?

    Call us Monday to Friday, 8 a.m. – 7 p.m., at 1-866-899-4828 / TTY / TTD 711. You can also call Medicaid at 1-877-254-1055 or click here, then choose “Report a Complaint".

  • When do I get my new member kit?

    Your New Member Packet should arrive within five (5) days after you are a member of Community Care Plan.

  • How do I know which doctor(s) I can go to?

  • How do I get Behavioral Health services?

    Please call Community Care Plan at 1-866-899-4828.

  • I have questions about my medication?

    Please call Prime Therapeutics at 1-800-424-7897, Monday to Friday, 8 a.m. – 7 p.m.

  • How do I get Vision Care?

    Please call iCare Health Solutions at 877-296-0799, Monday to Friday, 8 a.m. – 7 p.m

  • How do I get a ride to my doctor’s appointment?

    Please call ModivCare at 1-866-306-9358.

  • Are member materials in other languages?

    Yes, you can call us Monday to Friday, 8 a.m. – 7 p.m., at 1-866-899-4828 / TTY / TTD 711 to request.

  • How do I get a provider directory?

    You can receive a directory by calling us at 1-866-899-4828, or by visiting our website: www.ccpcares.org.

  • I have a new name / address / phone, what should I do?

    Please call the Department of Children and Families at 1-866-762-2237.
