At Community Care Plan, our mission is to build healthier communities.
And good health starts with you:
the voices that make up these communities.
In our Voices of the Community video series, it is our hope that voices
are heard by shining a light on the things that impact your health.
Introducing: Voices of the CommUnity
Maternity Care
Check back soon for new videos.
Voices are meant to be heard.
At Community Care Plan,
Our goal is to serve
What OUR community
Has to say.
Word for word.
As the health plan with a heart,
We focus on
All the facets and parts
that improve health care
From the start.
Housing.
Food.
Safety.
Money.
We are aware
Of all these things
That can impact your access to care,
And the quality there.
With our Voices of the Community video series,
It is our intention
To alleviate
The tensions
For the people
Impacted most
By health care inequity
And the factors
That do not get mentioned.
Like the mother
without maternity care,
Who is not equipped.
Like the neighbor
who needs behavioral health services.
Like the family
that needs help finding a safe and affordable place to live.
The voices
Community Care Plan wants to give.
These videos
Shine a light to help us see
And hear
The rhythm of how we breathe
The pulse of the people
The heart beat
And all that happens
In between.
The voices
Of our
community.
And Community Care Plan’s
Approach
To health care
Based on unity.
Together, we can make a difference.
Women are dying
And we’re trying
To make sense of it,
But cannot come to grips,
With the fact of it
White women are receiving different care
Than women of color
Seeking Maternity care
Who are 2 to 3 times more likely to die
From pregnancy-related reasons
Or during childbirth
Than their white counterparts
And to make matters worse,
Beyond birth,
Black babies are most likely to die
In their first year of life
It’s time to change these trends,
Together,
That’s why we are offering
Birth, Baby, and Beyond
A program for moms
Giving them the resources they need
For a healthy pregnancy.
Like Asia,
Who at age 16
Joined at week 14
of pregnancy
Combating depression and anxiety
In need of parenting resources
And to be part of a community
In Community Care Plan,
The health plan with a heart,
She found a team,
Received supplies,
Maternity care,
Giving her and her baby a healthier start
We are also providing
Health care
Providers
Implicit bias training.
Helping health care
Be more fair
Making sure
Everyone
Gets the same care.
