South-Florida-based Community Care Plan (CCP), the health plan with a heart, is pleased to announce that the Florida Agency for Health Care Administration (AHCA) has awarded a contract to CCP for its Statewide Medicaid Managed Care (SMMC) program. CCP was selected for Managed Medical Assistance (MMA) Plus, including the serious mental illness population for five regions (E, F, G, H, and I), which includes Orange, Osceola, Brevard, Seminole, Palm Beach, Indian River, Martin, Okeechobee, St. Lucie, Collier, Charlotte, Desoto, Glades, Hendry, Lee, Sarasota, Broward, Miami-Dade, and Monroe counties. The new contract is for a six-year term beginning February 2025.
CCP has managed the Medicaid benefits for Florida Medicaid beneficiaries since 2000 and is currently a managed care plan with a Florida Medicaid contract in Broward County. The new SMMC contract will expand CCP’s Medicaid geographic footprint from one to 19 Florida counties and CCP anticipates its membership will more than double.
"Community Care Plan is thrilled to have been selected by AHCA to expand our reach and serve Medicaid members in multiple regions as well as a specialty population," stated Jessica Lerner, President & CEO of Community Care Plan. "This is a significant milestone in our vision to ensure that every community has access to equitable, high-quality, affordable healthcare. CCP has been steadily preparing for growth over the last few years, and with the dedication of our employees, providers, community partners, and vendors, we know that we will continue to provide the exceptional and personal customer service that we provide today."
CCP has an extensive network of providers but welcomes providers from regions beyond Broward County to join our network. To begin the process of becoming a CCP provider, please visit the Become a Provider (ccpcares.org) on our website.
