Providers for PPUC

To streamline operations, Broward Health partnered with Community Care Plan to administer the health benefits for Broward Health’s Physician Payment for Uncompensated Care (PPUC) program.

Community Care Plan is:
  • Owned by Broward Health and Memorial Healthcare System
  • A Member of Florida Association of Health Plans (FAHP) and the Association of Community-Affiliated Plans (ACAP)
  • Fully accredited by National Committee for Quality Assurance (NCQA) for 3-years on our first attempt
  • Fully accredited by Accreditation Association for Ambulatory Health Care as a Health Plan
Provider Changes and Updates

Providers should notify CCP of the following administrative changes 60 days prior to the change, including:

  • Change of location, mailing address, or phone number
  • Change in Tax ID
  • Change in practice name
  • Practice closing
  • Adding/Terminating a provider from the practice
  • Adding/Closing a service location
  • Changes in a provider’s hospital privileges
  • Termination from Medicaid, license suspension or termination, and exclusion from participation in federally funded programs
  • Changes to NPI
