Community Care Plan (CCP), the health plan with a heart, is pleased to announce that we have been awarded a contract to provide subsidized Children’s Health Insurance Program (CHIP) and full-pay health insurance coverage through the

Beginning January 1, 2020, CCP will provide coverage in the following eight Southeast and South Florida counties:

  • Broward
  • Indian River

  • Miami-Dade
  • Monroe

Headquartered in Sunrise, Florida, CCP is owned by Broward Health and

To provide the best quality of care to our members, including future potential members, our Provider Operations Representatives will be contacting providers in the next few weeks to discuss CCP’s contracting process for the Florida Healthy Kids program.
