  • How or where can I get my ID card?

There are a few ways to get your ID card:

  • View your ID card in MyChart, our secure member portal that is available online or mobile app. If you want a physical card, you can also send us a message through MyChart.
  • Call our Welcome Team at 1-866-384-2926 and we will have one mailed to you.
  • Email us at ccp.customersvc@ccpcares.org and we will have one mailed to you.
  • How do I change my Primary Care Provider (PCP)/doctor?

There are a few ways to change your assigned PCP:

  • Send the change through MyChart, our secure member portal that is available online or mobile app.
  • Call our Welcome team at 1-866-384-2926. We can also help you find another PCP.
  • Email us at ccp.customersvc@ccpcares.org.
  • My name, address, and/or phone number has changed, what do I need to?
  • To change your name or address, please call DCF at 1-866-762-2237.
  • To change your mobile phone number, email, or how we contact you, please call our Welcome Team at 1-866-384-2926 / TTY / TTD 1-855-655-5303.
  • Why do I need to answer the Health Risk Assessment or “HRA”?
  • The HRA is a survey that helps us take better care of your health. The HRA only takes a few minutes, and your answers are kept confidential.
  • Complete the HRA within 45 days of the enrollment effective date (listed on the ID card) and you can earn a $15 gift card!
  • You can complete the HRA:
  • By paper with the HRA that was mailed to you in your welcome kit. When you are done, please return it with the free, postage-paid, self-addressed envelope that was also in the envelope.
  • Visiting our HRA webpage and submitting your information.
  • Using the MyChart website or mobile app any time.
  • Is my doctor or specialist In Network? How can I find out?
  • You can find our in-network doctors in our Provider Directory. You can also download or print the directory.
  • If you are unable to find the doctor you are looking for, please call our Welcome Team 1-866-384-2926 / TTY / TTD 1-855-655-5303 and they can help you find a doctor or provider. They can also update your assigned Primary Care Provider (“PCP”).
  • Where can I find what my plan covers or what my benefits are?
  • To learn about your plan and benefits, please review the Member Handbook. You can view, download, or print the handbook.
  • If you have questions about your benefits or would like a copy of the benefit handbook mailed to you, please call our Welcome Team at 1-866-384-2926 / TTY / TTD 1-855-655-5303.
  • Do you provide Dental benefits?

No, we do not provide dental benefits. Medicaid assigns a dental provider to you. If you do know what dental provider you have been assigned to, please contact Medicaid at 1-866-762-2237.

  • How long will my coverage last?

You are currently active with CCP. To get more information about how long your Medicaid coverage is valid for, please call DCF 1-866-762-2237.

  • What if I have other questions?

You can also contact us. We are open Monday – Friday 8:00am-7:00pm.

  • New members who have not spoken to our Welcome Team, can call us at 1-866-384-2926 / TTY / TTD 1-855-655-5303.
  • If you have already spoken to our Welcome team, you can call our Member Services Team at 1-866-899-4828.
