In 2021, during the COVID-19 pandemic, CCP created Heart, which stands for Health, Education, Access, Resources, and Tools, as a virtual center. Three years later, with the help of our partners in the CCP led-Broward Black Maternal Health Collaborative, Broward Health, Urban League of Broward County, and Broward County Healthy Start Coalition, and with $850,000 in funding from the Health Foundation of South Florida, we are opening the doors to our brick-and-mortar center in Lauderdale Lakes, Florida.

With 10-20% of a person’s health-related to medical care and 80-90% related to their health-related social needs, CCP wanted to truly impact one’s health by integrating clinical care and social needs. Additionally, Broward County has some of the poorest obstetrical outcomes in Florida, which are driven largely by health-related social needs such as housing, food, and financial insecurity, as well as low health literacy. A primary factor for choosing to have the center in the 33311-zip code was that it has a higher-than-average rate of preterm births and babies entering the NICU. This zip code is an OB/GYN desert, with limited physicians serving the entire pregnant population. The center is an opportunity to bring additional pregnancy-related care into this community. By establishing a physical presence in an area of Broward County with the highest needs and poorest outcomes, CCP, in partnership with existing community-based organizations will bring the services, resources, and programs intended to improve social risk and OB outcomes, therefore, improving the health of the community.

The center also includes a Broward Healthpoint Maternity Care Center that offers CenteringPregnancy (group prenatal care), family planning, routine prenatal care, prenatal care and ultrasound, and telemedicine.

The center held its ribbon cutting on Friday, April 19, 2024, and its grand opening on Saturday, April 27, 2024.
